Please read the following terms and conditions carefully before selling your test strips and supplies. By shipping your supplies to us, you signify your agreement with these terms and conditions. 

1. I understand I can only sell supplies that are authentic, factory sealed, relatively undamaged, unexpired (a full 5 months before expiration date) and have been properly stored per manufacturer specifications otherwise I am subject to a %50 discount/rejected item on my order.
2. I am the rightful owner or have been given permission by the rightful owner to sell these supplies. Furthermore, I signify that I did not, am not and will not violate any applicable law or regulation in obtaining these supplies, nor in selling them to TestStripBank.com. I represent and warrant that my sale and transfer of these supplies to TestStripBank.com will not cause TestStripBank.com to, or result in, TestStripBank.com violating any anti-money laundering, anti-terrorism, healthcare, or any other applicable law.
3. I understand I cannot sell supplies that are prohibited for sale/distribution in the U.S., have undergone liquid damage, have been tampered with, have been improperly stored per manufacturer specifications or have been otherwise altered.
4. If supplies do not meet all the aforementioned terms, and/or are ones that TestStripBank.com does not accept, I understand that my test strips will be discounted,returned,discarded, or donated. 
5. If supplies meet all of the aforementioned terms and conditions, I understand that my payment will be dispersed within 2 business days of receiving my package and typically arrive 5-9 business days thereafter, but may take longer depending on mail speed.
6. I am responsible for my package during shipping and TestStripBank.com is not responsible for packages that are lost or damaged by the U.S. Postal Service.
7. I understand that shipping times vary and while most packages are delivered within a week, some packages may take up to 14 days.
8. TestStripBank.com will destroy all prescription labels before redistributing my supplies.
9. I understand that I cannot intentionally acquire supplies through my insurance for the purpose of selling them to TestStripBank.com, nor sell products that were paid for by a government sponsored plan such as Medicare/Mediaid.
10. I agree to transfer all right, title and interest in the supplies to TestStripBank.com upon the receipt of the supplies by TestStripBank.com, and I warrant that I have the legal right to do so.
11. I understand that TestStripBank.com has not made any representation that the value assigned to the item is equivalent to fair market value.
12. I understand that the terms and conditions may change at any time and I will not be notified of these changes. I understand it is my responsibility to read through the terms and conditions which can be found on TestStripBank.com/terms-and-conditions every time I sell my diabetic supplies to TestStripBank.com.
13. I understand the prices quoted to me by TestStripBank.com and all of the applicable policies stated herein. It is incumbent on me to familiarize myself with the TestStripBank.com’s pricing structure and policies before sending in my strips, and I understand that by sending products to TestStripBank.com I forfeit my rights to said products.
14. I am at least 18 years old.
